Existing business with either a Gambling license or Liquor license
For existing businesses who have either a Gambling license or a Liquor license, you may use our abbreviated Supplemental License form. This form must be signed during an in-person conversation with a sales representative.

- Owner(s) and spouse(s) of Sole Proprietors
- Partner(s) and spouse(s) of each Partnership

Personal/Criminal History forms may not be required if the organization is currently licensed with the Lottery for another business.

Personal/Criminal History forms must be completed by:
Officers, members/managers, any authorized signatory of documents and any 10% or more stockholder(s). NOTE: If corporate officers are located out of state, a corporation may designate other local corporate approving authority, such as a mall manager, for criminal and credit history investigation. Acceptance of non-corporate officers for background check investigation, in lieu of corporate officers, is subject to the Director’s approval.
Personal/Criminal History forms may not be required if the organization is currently licensed with the Lottery for another business.
Complete the Process
All retailers who are licensed must sign a Retailer Contract to complete the establishment of a business relationship with Washington’s Lottery.

We install wireless communications equipment at 98% of all retailer locations. You will need to have the building owner sign a wireless installation permission form for us to install the communication equipment.
